FMH Hospice mission: to provide superior and compassionate care to patients and their loved ones, so they can complete their life journey in comfort with dignity and harmony.   The Hospice RN Case Manager is a vital member of the interdisciplinary team who work together to meet the physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and families facing terminal illness and bereavement. The Hospice RN Case Manager is designated to provide coordination of care, ensure continuous assessment of each patient and family’s needs and implementation of the interdisciplinary plan of care.   As a Hospice Registered Nurse Case Manager (RN), you will: - Oversee the implementation and ongoing assessment of the patient’s plan of care alongside the spiritual care coordinator, social worker, hospice aide and medical director.      - Ensure an accurate evaluation and work to develop and implement a comprehensive care plan that addresses patient needs.     - Communicate patient conditions, collaborate with appropriate providers, and team members, to deliver care as patient needs evolve.       About Fairbanks Memorial Hospital Fairbanks Memorial Hospital is a non-profit facility owned by the Greater Fairbanks Community Hospital Foundation. In Fairbanks, small-town living, spectacular natural beauty, and endless recreation combine to create a one-of-a-kind place to live, work and play.   History of Hospice in Fairbanks In 1988 a volunteer-based hospice was started in Fairbanks known as Hospice of the Tanana Valley. In 2012 Hospice of the Tanana Valley merged with Fairbanks Memorial Hospital to become a medically based hospice. Our Hospice covers a 26-mile radius in the Fairbanks North Star borough. Most hospice patients are cared for in their own homes or the homes of a loved one. Home may also be broadly construed to include services provided in nursing or assisted living facilities, wherever the patient considers to be home.   Position available: Monday-Friday (5) 8hr days or (4) 10hr days. Share of on-call expected, including weekends, nights, and holidays. Mileage reimbursement   *Per Diem also available*   Qualifications: - Education: Associate Degree in Nursing (ADN) or; Bachelor of Science in Nursing (BSN) preferred.    - Licensure: Current license to practice as a Registered Nurse (RN) in the state of Alaska.    - Valid state-issued driver’s license, automobile insurance, and reliable transportation. - One (1) year of professional experience practicing as a Registered Nurse (RN) in a hospice or similar setting preferred

The position is primarily accountable for performing initial clinical appropriateness reviews of requests for services that require prior authorization, initial, concurrent and/or retrospective review The review process is performed in accordance with established patient (health plan) protocols/guidelines, benefits, and clinical appropriateness criteria (clinical screening criteria). This position works closely with the Care Coordinators, Discharge Planners, Social Workers, Registration and Providers to evaluate, monitor, and assure the appropriateness and medical necessity of selected care for patients as it relates to quality, continuity, and cost-effectiveness. This position works with clinical and non-clinical staff as part of the referral and/or authorization process. This position is responsible for managing utilization and, if necessary, discussing the utilization of service and standards of patient management.   About Fairbanks Memorial Hospital Fairbanks Memorial Hospital is a non-profit facility owned by the Greater Fairbanks Community Hospital Foundation.
