    Records medical details, gathers data, and coordinates tests, orders and results as necessary to ensure an accurate and complete medical record. Assembles and maintains patients' health information in medical records and charts by assisting medical providers and physicians. No medical decision making is performed.

    Position Available

    Full-time, 40 hours/week, Monday - Friday, occasional overtime, occasional weekends required. 

    Responsibilities

    ESSENTIAL FUNCTIONS

    1. Records details of chief complaint, history of present illness, review of systems, past medical history, social history, family medical history, disease risk factors, medications and allergies in to the medical record system.  
    1. Accompanies physician into patient examination area in order to transcribe a history and physical examination as given by the patient and physician or medical provider.  
    1. Records details of physical exam, including pertinent positive and negative noted abnormalities and patients’ general condition in the medical record system.  
    1. Validates test results are received and reviewed during patient visit as directed by medical provider.  
    1. Coordinates tests, orders, and results to ensure all health information is recorded accurately and ready for access by the medical provider.  
    1. Lists all proper diagnoses as well and any discharge/follow up instructions and prescriptions as dictated by the physician or medical provider.  
    1. Maintains confidentiality of all patient information.  
    1. Ensures that provider has signed their own dictation and super-bill.  

    Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards.     Qualifications

    MINIMUM QUALIFICATIONS

    High school diploma/GED or equivalent working knowledge and strong understanding of medical terminology typically achieved by completing a course in medical terminology, anatomy, medications.

     

    Individuals must have ability to obtain certification as a Medical Scribe (CMSS, CMSA) within six months of employment and be credentialed through the Medical Scribe Certification & Aptitude Test (MSCAT), thus complying with CMS, as attested through the Certified Medical Scribe Specialist (CMSS) or Certified Medical Scribe Apprentice (CMSA) clinical duties and credential.

     

    Current BLS certification. All direct patient care personnel are required to demonstrate competent performance of cardiopulmonary resuscitation (BLS) techniques for adults, children and infants according to the standards of a nationally recognized, standardized, credentialed healthcare provider course every other year.

     

    Requires skills and abilities typically attained with 1 year of medical clerical experience, preferably in a hospital or physician office setting, or prior experience in customer service. Must possess excellent organizational skills as well as effective human relations and communication skills. Working knowledge of medical terminology and computer literacy and keyboarding skills is required.

     

    PREFERRED QUALIFICATIONS

    Preferred candidates have coursework related to obtaining a bachelor of science in pre-med studies, biological sciences, or related field.

     

    Two years of experience in acute care or multi-specialty clinic using electronic medical record software or maintaining medical records.

     

    Additional related education and/or experience preferred.

     

    PHYSICAL DEMANDS/ENVIRONMENT FACTORS

    IP - Typical Indirect Patient Care Environment: (CSPD Instrument Technician, Lab Technician, Pharmacist)

    • Able to stand, walk, bend, squat, reach, and stretch frequently.
    • May be required to lift up to 75 pounds.
    • Must use standard precautions due to threat of exposure to blood and body fluids.
    • Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone.
    • May require periodic to frequent use of personal computer.
    • Off-site travel may be required.
