Overview

Foundation Health Partners (FHP) is looking for a Medical History Pharmacy Tech to join the Inpatient team!

 

This position works under the supervision of a pharmacist to obtain, verify, and document medication histories for patients presenting to the Emergency Department and/or admitted to Fairbanks Memorial Hospital. Interviews patients, families, and caregivers and reviews available information from pharmacies, healthcare providers, care facilities, and medical records. Documents findings in the electronic health record and refers discrepancies or incomplete information to the pharmacist for review. Serves as a resource for medication history processes and supports related training and quality-improvement activities.

 

*Requires State of Alaska Pharmacy Technician License and Pharmacy Technician Certification 

 

Pay & Benefits:

  • Compensation: $28.47 to $44.49 hourly wage based on experience and education

Responsibilities

  • Conducts structured interviews with patients, families, and caregivers to obtain complete medication histories, including medication name, dose, route, frequency, indication, last dose, and reported adherence.
  • Verifies reported medication information using available pharmacy records, medical records, provider offices, care facilities, and other approved sources. Documents verification sources according to department standards.
  • Documents medication histories in the electronic health record within established timeframes. Identifies histories as complete, partial, or unobtainable and records the reason when information is incomplete.
  • Identifies discrepancies, conflicting information, and medication-related concerns and refers them to the pharmacist for review. The role does not provide clinical interpretation, therapeutic recommendations, or medication-reconciliation decisions.
  • Coordinates medication history queues and handoffs with pharmacy staff and contracted medication history services.
  • Participates in quality reviews related to the accuracy, completeness, and timeliness of medication histories. Completes assigned follow-up actions.
  • Serves as a medication history resource and supports the training and orientation of pharmacy technicians and other staff.
  • Protects patient privacy, verifies patient identity, and maintains accurate documentation.
  • Maintains competency in core pharmacy technician functions and provides department coverage as assigned.
  • Other duties as assigned.

Qualifications

  • Requires pharmacy technician licensure in the state of Alaska upon hire. Pharmacy technician certification required (CPhT or ExCPT).
  • Must demonstrate two (2) years of experience performing in a pharmacy technician role in a hospital, ambulatory, or community setting, or knowledge typically obtained within two (2) years of experience.
  • Must possess good organizational, problem solving, math, and computer skills. Must have good written and verbal communication skills for interfacing with all levels of staff, physicians, patients, and other contacts.
  • Demonstrated ability to conduct a structured patient interview, elicit accurate information from patients and caregivers under time pressure, and document findings clearly in an electronic health record.
  • Must have knowledge of computer software, particularly medication management information system software, Microsoft applications, and automated dispensing systems.

Preferred Qualifications

  • Prior experience obtaining medication histories or supporting medication reconciliation in an emergency department, admissions, or transitions of care setting.
  • PTCB Medication History Certificate or equivalent formal training in medication history collection.
  • Pharmacy technician experience and supervisory experience preferred. Additional training in specialty areas preferred.
  • Bilingual ability, or experience working with interpreter services.
  • Additional related education and/or experience preferred.
