Position Overview:
As our Inpatient Prenatal Care Navigator, you will be a key member of our team, helping to bridge the gap between outpatient and inpatient care. You will play a vital role in ensuring that every patient receives timely, personalized, and comprehensive prenatal care by collaborating with the interdisciplinary team. Your ability to extract valuable data from prenatal records, identify potential risks, and streamline patient transitions will make a direct impact on the health and well-being of both mothers and their babies.

Coordinate seamless care transitions between clinic and hospital settings, ensuring timely and well-organized care.
Review and document prenatal records in the EMR to maintain accurate and complete patient information.
Identify and address prenatal risk factors early, collaborating with providers and interdisciplinary teams to optimize care plans.
Advocate for and support patients, helping expectant mothers feel valued, informed, and empowered.
Contribute to quality improvement efforts that enhance workflows, communication, and patient satisfaction.
Provide bedside support during times of high acuity or increased unit activity.
Shift Schedule:
Monday - Friday
Dayshift Clinic Hours.
Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Provides all customers of Foundation Health with an excellent service experience by consistently demonstrating our core and leader behaviors each and every day.
Must possess a valid current or temporary Alaska RN license. Requires the knowledge and skills normally demonstrated by an Associate's degree in nursing. BLS certification is required. May require ACLS, PALS, or additional certification and/or continuing education based on area of practice.
Requires creative thinking, prioritizing, and flexible problem solving skills. Also requires the ability to communicate clearly in both verbal and written forms.
PREFERRED QUALIFICATIONS
Bachelor's degree preferred. Professional certification preferred.
Experience working in an acute care setting within the past 12 months as a Registered Nurse in the specialty area.
Additional related education and/or experience preferred.
