Overview

This position provides skilled patient care to promote patient comfort and contributes to the overall patient plan of care. This position is responsible for on-going patient data collection and documentation in the medical record; provides a variety of unit support functions which contribute to the overall efficient operation of the nursing unit/department. This position acts as a lead to the nursing unit, trains and mentors other employees, as well as oversees the quality of care of those within the unit. This position serves as a resource for non-licensed staff, provides a variety of unit support functions, and contributes to the overall efficient operation of the department.

 

Must possess a minimum of one (1) year of C.N.A. experience.

 

  • Compensation: $20.07-$28.90 per hour based on experience and education

Responsibilities

  1. Provides daily living assistance and completes necessary tasks to promote patient health. Assists patients with basic hygiene needs, maintains optimum mobility levels, provides safe patient handling, O2, infection control practices, assists patients with nutritional intake, as needed, and provides end of life care.
  1. Collects patient specimens and data, including vital signs, glucose testing, input/output and other delegated measurements in accordance with unit/facility policy and standards of care. Documents objective data and routine aspects of patient care. Assists in the transportation of patient specimens.
  1. Recognizes and reports abnormalities and/or changes in to the patient's health status, as well as patient health responses from treatment of a licensed professional. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for which care is provided.
  1. Performs procedures and uses equipment as delegated by a licensed professional, consistent with demonstrated competencies and state scope of practice; procedures may include blood draws, Foley catheter removal, EKGs, EKG monitoring, and bladder scanning. Provides necessary supplies, support and assistance to medical staff and patients for unit specific procedures, under the direct supervision of a licensed professional.
  1. Maintains a clean, orderly and well-stocked environment. Understands and able to incorporate long term care infection control standards.
  1. May provide general support to the department, including greeting and assisting patients, visitors, providers, and ancillary personnel. May escort patients to appointments and planned outings, assuring they arrive in a safe and timely manner.
  1. May assist as needed with general receptionist and clerical duties in order to facilitate efficient functioning of the unit/department.
  1. Mentors, trains, and teaches other staff. Involved in performance improvement projects, incorporates the Eden philosophy with residents and staff, and maintains additional competencies in Alzheimer's and Dementia support 

Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Provides all customers of Foundation Health with an excellent service experience by consistently demonstrating our core and leader behaviors each and every day.

Qualifications

High school diploma/GED or equivalent working knowledge is required.

Must possess a minimum of one (1) year of C.N.A. experience. Requires proficiency in all competencies including end of life care, Alzheimer's, and Dementia. The ability to train and mentor other employees is required. The ability to float to other areas of the facility is required. Preceptor education course required.

Current BLS certification. Current C.N.A. license in state of practice is required in accordance with regulatory requirements or facility/department policy. Demonstrates critical thinking skills and ability to lead other employees

Must possess excellent organizational skills, as well as effective human relations and communication skills. Working knowledge of medical terminology and computer literacy and keyboarding skills is required.

As is typical in this industry, variable shifts and hours, and on-call duties may be required.

PREFERRED QUALIFICATIONS
Additional related education and/or previous leadership experience preferred.
