The Hospice RN Case Manager is a vital member of the interdisciplinary team who work together to meet the physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and families facing terminal illness and bereavement. The Hospice RN Case Manager is designated to provide coordination of care, ensure continuous assessment of each patient and family’s needs and implementation of the interdisciplinary plan of care.

History of Hospice in Fairbanks
In 1988 a volunteer-based hospice was started in Fairbanks known as Hospice of the Tanana Valley.
In 2012 Hospice of the Tanana Valley merged with Fairbanks Memorial Hospital to become a medically based hospice. Our Hospice covers a 26-mile radius in the Fairbanks North Star borough. Most hospice patients are cared for in their own homes or the homes of a loved one. Home may also be broadly construed to include services provided in nursing or assisted living facilities, wherever the patient considers to be home.
Position available:
Monday-Friday (5) 8hr days or (4) 10hr days. Share of on-call expected, including weekends, nights, and holidays.

Qualifications:
Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. Provides all customers of Foundation Health with an excellent service experience by consistently demonstrating our core and leader behaviors each and every day.
Must possess a valid current or temporary Alaska RN license. Requires the knowledge and skills normally demonstrated by an Associate's degree in nursing. BLS certification is required. May require ACLS, PALS, or additional certification and/or continuing education based on area of practice.
Requires creative thinking, prioritizing, and flexible problem solving skills. Also requires the ability to communicate clearly in both verbal and written forms.
PREFERRED QUALIFICATIONS
Bachelor's degree preferred. Professional certification preferred.
Experience working in an acute care setting within the past 12 months as a Registered Nurse in the specialty area.
Additional related education and/or experience preferred.
